In order to receive an infant check-up, you need to fill out a check-up sheet. You can fill it out at the hospital, but you can fill out the infant check-up questionnaire in advance on the website of a healthy person.
Also, be sure to call the hospital where you want to receive infant check-up before visiting.
There are some hospitals where you can go to the hospital and fill out a questionnaire to receive an infant check-up, but you may not be able to receive an infant check-up when you visit on the same day. This is because some hospitals cannot receive infant screening without prior reservation.
In addition, you need to fill out a questionnaire before screening for infants and toddlers. If you fill out the questionnaire in advance on the ‘Healthy Person’ site and visit the hospital, you can receive the checkup faster.

In the center of the main screen of the home page of healthy people, click “Create Infant Questionnaire/Developmental Screening” marked with a pink square.

Log in to “Guardian’s Authorized Certificate” – “Search for Children Subject to Screening”.

Application for questionnaire reading service (before filling out questionnaire)
There are a few things you need to do before you hit the Create Questionnaire button.
If you press the Create Questionnaire button, a pop-up window will appear saying “Please apply for the questionnaire reading service”.
At first, I was at a loss for the question ‘Where should I use the questionnaire reading service?’
To apply for the medical questionnaire reading service, first click on the “Service Application” in the pink square.
Agree to application for questionnaire reading service, set password
Receive registration password text message
If you click “Apply for service”, a pop-up window for requesting a questionnaire reading service opens, check the agreement, and enter your registration (password) number.
If you enter a 4-digit password and your mobile phone number, you will receive a text message of the registration (password) number.
f you have done this far, you can now fill out the infant screening questionnaire and developmental screening questionnaire in order.
You can also visit the hospital where you will be screened for infants and toddlers.
Some hospitals require reservations for infant screening, so be sure to call before visiting.
